A few things:

Please ensure that you have contacted Ian Benjamin Psychology either by phone or email before making your first appointment so that a Client Registration form and an Informed Consent form can be emailed to you - submission of these forms is required.

For Medicare and DVA consumers, please arrange a referral from your GP before your first appointment if you wish to claim a Medicare rebate or to be bulk-billed (for DVA consumers and concessionally eligible Medicare consumers).

 

From July, 2026, if you are a new consumer seeking to be bulk-billed through Medicare, you will be sent a form requesting you provide block* assignment of your Medicare benefit to Ian Benjamin Psychology (*covering the first block of 6 appointments, and again for the remaining block of 4 appointments your referral allows, if necessary). The form for the first block (of 6 appointments) must be completed before treatment begins. 

For SIRA (Workers Compensation or CTP Insurance) consumers, please make sure you have contacted your insurer and have a claim number, as well as a GP referral (Certificate of Capacity).
