Ready to start?

Please ensure that you have contacted Ian Benjamin Psychology either by phone or email before making your first appointment so that a Client Registration form and an Informed Consent form can be emailed to you - completion and submission of these forms is required.

For Medicare and DVA consumers, please arrange and provide a referral from your GP before your first appointment if you wish to claim a Medicare rebate or to be bulk-billed (for DVA consumers and concessionally eligible Medicare consumers).

For SIRA (Workers Compensation or CTP Insurance) consumers, please make sure you have contacted your insurer and have a claim number, as well as a GP referral (Certificate of Capacity).
